The following documentation shall qualify as evidence of liability insurance coverage:

  1. A copy of the certificate of insurance as issued by the insurance provider.
  2. If the certificate of insurance does not identify the insured midwife by name, a letter stating the name of the insured registered midwife, printed on employer letterhead and signed by the employer, shall accompany the certificate of insurance.
